What do I do first ?
You start by getting an ADP Hearing Device Application form. You can get this form from an authorizer or vendor registered with ADP, or by calling :

Toll Free : 1-800-387-5559

Do I fill in the entire ADP form ?
No. You fill in only the first part called Section One.

The other parts of the form are completed by your physician or an audiologist, the ADP authorizer, the ADP dispenser and the ADP registered vendor.

What happens next ?
You must have a hearing test before being fitted for a hearing aid. There may be a charge for this test. Ask about this before you make an appointment for your hearing test.

Who can test my hearing and fit my hearing aid ?
You may choose one of the two ways to get this done :

Go to a doctor, preferably a hearing specialist. The doctor will fill in Section Two of your ADP form to verify your need for a hearing aid.
Then go to an authorizer/ dispenser who is registered with ADP. The authorizer/ dispenser will test your hearing. He or she will write the exact type of hearing aid and accessories in Section Three of the ADP form and fit your hearing aid.

OR

Go to an audiologist who is a registered authorizer with ADP. The audiologist will test your hearing and fill in Section Two and Three of your form.
Then go to a dispenser registered with ADP who will order and fit your hearing aid.

Who sells me the hearing aid ?
You must buy your hearing aid from a vendor registered with ADP. You will be fitted for the aid by a dispenser who is also registered with ADP.

The registered vendor bills ADP for the amount it will pay towards your hearing aid. The registered vendor then charges you the remaining cost.

How much does ADP pay for my hearing aid ?
ADP will pay the registered vendor 75 per cent up to a maximum of $500 of the cost of the hearing aid, ear mould, accessories listed with ADP, and the dispensing fee.

If you need hearing aids for both ears, the vendor will bill ADP for 75 per cent up to a maximum of $1,000 of the cost of two hearing aids, two ear moulds, accessories listed with ADP, and two dispensing fees.

The ear mould must be bought at the same time as the hearing aid. ADP will not contribute to the cost of ear moulds bought separately from hearing aids.

The registered vendor will charge a dispensing fee to fit the hearing aid. The dispensing fee may vary depending on the type of hearing aid.

For FM systems, ADP will pay the registered vendor 75 per cent up to a maximum of $1,350 of the cost of the ADP listed device and dispensing fee.

Is there a trial period for me to try out my hearing aid ?
Yes. You have 30 days to try out your hearing aid. If you are not satisfied, return the hearing aid to the vendor before the end of this trial period.

The vendor may charge you a handling fee.

Is my hearing aid covered by a warranty ?
Yes. A manufacturer's minimum one-year warranty comes with every hearing aid. However, you are responsible for the cost of repairs when this warranty ends.

Can I replace my hearing aid if my medical condition changes ?
You must see a doctor who is a hearing specialist. ADP will contribute to the cost of a new hearing aid after the doctor explains the need for it on an ADP form and ensures you meet the replacement requirements.
